Recertification Credits

UPMA 2020 Spring Conference Recertification Credits 
February 6-7, 2020 – Marriott University Park, Salt Lake City, UT

To record your participation in the educational sessions, use the UPMA Verification of Attendance Form unless stated differently by your state. State forms will be located on the table as you enter the educational session room.

Things to keep in mind to ensure that you receive credit for the approved sessions:

  • Read all documents carefully.
  • You MUST attend the entire session.
  • The UPMA Verification Form MUST be punched at the end of each attended session. STATES DO NOT ALLOW THE FORM TO BE PUNCHED AT ANY OTHER TIME.
  • If your state requires the UPMA Form, please leave the white and yellow copy with the UPMA monitor, UNLESS YOUR STATE REQUIRES YOU TO SEND THE ORIGINAL.
  • If your state requires a course number, you MUST enter it on the UPMA Verification Form.

Please follow the directions below for your state(s):

CO

Have the UPMA form punched. Be sure to include Course #20191 on your verification form. Use a personal mailing address on the form NOT the address of the company. Sessions 1, 2, 3, 4, 6, 7, and 10 have qualified for credit. The following credits have been awarded:

***Licensed qualified supervisors and certified operators may obtain TWO (2) Continuing Education Credits in Laws and Regulations by attending AT LEAST two of the Laws and Regulations sessions listed above. A maximum of two Laws and Regulations credits will be given. If attendance at both sessions is not documented on the verification form, then only one credit will be given.

***Licensed qualified supervisors and certified operators may obtain TWO (2) Continuing Education credits in Residential/Commercial Pest Control by attending AT LEAST two of the Residential/Commercial Pest Control session listed above. A maximum of two Residential/Commercial Pest Control credits will be given. If attendance at both sessions is not documented on the verification form, then only one credit will be given.

Have the UPMA form punched.  Please give the white and yellow copies to the UPMA staff and keep the pink copy. You must include course # 2020183-7 in the course numbers box. February 6th sessions have been approved for a total of 7 credits. No sessions on February 7th have been approved credit. At the end of the conference, you must pick up an Idaho Department of Agriculture sticker from the registration desk. You are required to place this sticker on the back of your license.

Have the NPMA form punched. Please include the course number NV-20-051 on your form. The maximum number an attendee can earn is 10 CEUs (3 Law & 5 General). The following sessions have been approved for credit.

UT

Have the UPMA form punched.  Attendees are required to keep the white & yellow copies of the punch form & turn the pink copy into UPMA staff. The attendee is required to keep the white copy of the punch form with all other recertifications paperwork submit to the state at the time of the license renewal. In the Course Number box please list the session, category, and credit amount awarded based on the table below.

Have the UPMA form filled out and punched after each session attended. You MUST include course number #1279 in the Course Numbers box on the UPMA form. This conference has been approved for 10 credits. Turn the white & yellow copies of the UPMA form into the UPMA staff and keep the pink copy for your records.
