Recertification Credits - UPMA 2021 Fall Conference

 

To record your participation in the educational sessions, use the specific state’ attendance forms located on the table as you enter the educational session room.

Things to keep in mind to ensure that you receive credit for the approved sessions:

  • Read all documents carefully.
  • You MUST attend the entire session.
  • The IPCA Verification Form MUST be punched at the end of each attended session. STATES DO NOT ALLOW THE FORM TO BE PUNCHED AT ANY OTHER TIME.
  • If your state requires the IPCA Form, please leave the white and yellow copy with the IPCA monitor at the registration desk, UNLESS YOUR STATE REQUIRES YOU TO SEND THE ORIGINAL.
  • If your state requires a course number, you MUST enter it on the IPCA Verification Form.
  • You must fill out a new form for each state you need recertification credits for. Please follow the directions listed, on the following pages, for your state(s).

*** NOTE: Licensed qualified supervisors and certified operators may obtain TWO (2) Continuing Education Credits in Residential/ Commercial Pest Control by attending AT LEAST two of the Residential Commercial Pest Control sessions listed above. A maximum of two Residential/ Commercial Pest Control credits will be given. If attendance at both sessions is not documented on the verification form, then only one credit will be given.

The Utah Pest Management Fall 2021 conference has been approved for 8 credits. UDAF does not have a CEU tracking system. You do not need to add a course number to your form. Utah Private, Commercial, and Non-Commercial Pesticide Applicators must track their own CEUs. On site, keep pink copy of UPMA Verification Attendance form for your records. Give while and yellow copies to UPMA Monitor.
